Necessary Documents: Applicants must bring all necessary documents for the service requested. The list of documents needed for each service is published on our website: http://Curacao.USConsulate.gov. To avoid delays, please read the instructions carefully.

Payment: Payment must be made by cashier's check. The cashier's check must be in U.S. dollars, negotiable in the U.S., and made out to "U.S. Disbursing Officer." CASH AND PERSONAL CHECKS CANNOT BE ACCEPTED.
Forms and Instructions:
* See the chart above for guidance on which form to use.
* For regular passport renewals, applicants will use the form DS-82.
* Applicants using passport form DS-11 must appear in person.
* Passport applicants under 16 years of age must use the DS-11 passport form and must appear in person with both parents.
* Bring both originals and copies of the parents' passports and the child's birth certificate.
* If one parent cannot appear in person, a notarized statement of consent of renewal of the minor's passport (DS-3053) from the non-appearing parent is required.
* Bring photocopies of all original documents, including passports, for each application.
* Complete all necessary forms prior to arriving but do not sign the form until instructed by the Consular Officer.
Delivery of U.S. Passports and CRBAs: Applicants may arrange delivery of passports and CRBAs by submitting a pre-paid airway bill or account number from a courier service or by collecting the document in person at the U.S. Consulate General in Curacao.
